How to Join Us: To begin your journey with us, please take a moment to review the information below and complete the registration process:

Registration Form: Fill out our registration form and sign any necessary documents. You can either:
- Email the forms to us at enquiries@broadwaymed.co.nz
- Drop them off at the practice in person
Please ensure all signatures are hand-signed either by printing the form or using a stylus or mouse on your computer.

Documentation for Non-New Zealanders: If you were not born in New Zealand, please provide a copy of your passport and any relevant visa details.

Confirmation of Enrollment: Once we receive your completed forms and enrol you, you'll receive another email containing further instructions.

New Patient Medical and Health Questionnaire: You'll also receive a New Patient Medical and Health Questionnaire. We recommend scheduling a New Patient Appointment as soon as possible with one of our nurses, who will review your health information and introduce you to our practice. This appointment can be conducted in person or over the phone. Subsequently, you can arrange an appointment with a doctor if necessary.
