Patient Experience Survey

Patient experience survey. We want to hear from you! We want to know how you found your experience with doctors, nurses and staff in the health care system. The survey is voluntary, and anonymous. You can complete it at home, on a computer, tablet or smart phone. It takes about 15 minutes. You may get an email or text inviting you to take part.
